Now spine surgeons are thinking about how today's state of the industry will be looked back on in 2034.

In Becker's most recent Ask Spine Surgeons series, the publication spoke with top spine surgeons around the country to ask them how they will look at the current state of the field in 10 years.

"In 10 years, I believe spine surgeons will look back at the current state of the field and shake their head at the number of fusions performed instead of disc replacements. The same thing happened in 1958 when John Charley invented the artificial hip. Most surgeons at the time laughed at him and said his device would not work. That hip replacement was a fad. Instead, knee and hip fusion were standard of care. In 10 years the field will have shifted dramatically to motion preservation surgery — artificial disc replacement, facet joint replacement — to the point that we may see the ability to partially correct 20 degree to 25 degree scoliosis curves with motion sparing devices. Just like arthroplasty is now commonplace for hip and knee and fusion is rare, the same will be true of spine devices; cervical and lumbar artificial discs will be the mainstay of treatment and spinal fusion will be the rare exception."

The term “off-label procedure” relates to how

The term “off-label procedure” relates to how the FDA approves devices. Every implant must go through a clinical trial, and FDA approval follows a long-term study of each patient who receives the implant. For example, if the study is for one-level disc replacement, the disc will become approved for single-level ADR.

If a patient requires a three-level disc replacement, for example, it technically becomes considered “off-label” because that’s not the use that was explicitly studied in the trial. Off-label procedures are still allowed, and Dr. Lanman performs successful off-label procedures all the time.

It also tends to be a technicality that insurance companies lean on to avoid paying for surgeries that their policyholders need.

This patient is a 54-year-old male police detectiv

This patient is a 54-year-old male police detective who had an injury to his neck. He states that his pain started after an aggressive chiropractic manipulation. Seeking relief from this injury, he underwent a cervical spinal fusion in 2020. The fusion did not relieve his pain, but instead seemed to increase the pain he was experiencing in his right arm and right shoulder blade. In fact, he stated that he felt like there was a metal rod stuck through his neck. He could never get comfortable, regardless of how he held his neck or oriented his body. Of course, this made it nearly impossible for him to work as a detective.

I reviewed the patient’s neck x-rays and found that he was fused at C4-5, C5-6, and C6-7. The fusions had healed nicely, which doesn’t always happen. As expected, there was no movement in the fused segments, but, troublingly, the detective was unable to fully extend his neck. It other words, he really couldn’t look up to any practical degree.

No surgeon would operate on him because no one wanted to operate on a healed fusion. But the man was having debilitating pain and couldn’t extend his neck. I agreed to remove the plate and resect the fusions. I replaced the plate fusions with artificial discs at all three levels. If you look at his X-rays now, you can see how much farther he can flex and extend his neck. This doesn’t just give him a better and more functional range of neck motion, but it relaxes the musculature in his neck. He had virtually immediate relief of symptoms after the cervical fusion reversal. Two years later, he is still doing great.

He is just one success story. I have performed over four dozen reversals of cervical fusions where the placement of an artificial disc eliminated the patient’s pain syndrome. While additional research is needed to assess long-term effects, I’ve clearly shown that patients with significant pain after fusion can increase spinal mobility and find pain relief through cervical fusion reversal and artificial disc replacement—even if the fusion has healed.
